Advanced Calculator

I am looking for direction on how I can add a calculator to one of my Nimva themed pages. Need to add a calculator that can accept a couple of inputs from the page visitor, integrate with a database, and display results....much like what this gentleman offers: http://bossakov.eu/wpdemo/pvb-cf7-calculator-pro/

I don't mind purchasing his solution, but it doesn't work within the Nimva theme version I have (2.4).

Has anyone built something similar that they can give me some advice to follow? I have minimal understanding of php, java script, html, or any other web language....my logic is strong, my raw coding skills are not...that is why I purchased a Nimva theme as opposed to coding my page.
